Note: If one or more of the recipients on your Examination and Board Action History Report (EBAHR) form are not licensing entities you will NOT be able to use the on-line process. You will need to submit a paper-based EBAHR form with the proper notarizations. |
You may designate up to two (2) recipients** for each $50 fee as indicated in Section III, Part B of the online EBAHR request form, regardless of the number of examinations included on the report.
Once an online EBAHR request has been submitted, only the recipient(s) listed on that EBAHR will be processed; the fee can not be applied to an additional recipient requested at a later date. The Federation issues official transcripts within 5 business days of receiving the completed EBAHR request and appropriate fee. Express shipping to addresses within the US and its territories is available for an additional $20 per recipient. Most state medical boards receive official transcripts in electronic format via a secure website. For those boards that do not receive transcripts electronically, the $20 ensures express shipping of the transcript. Express service does not deliver to PO Box addresses. All other transcripts are sent via first class mail.
** 1-2 recipients = $50; 3-4 recipients = $100; 5-6 recipients = $150; etc.
