The online Common License Application Form (CLAF) is a new licensure application process for physicians. CLAF has been developed to: 1) reduce the number of incomplete applications received by state medical boards, 2) allow for the collection of uniform information, and 3) add convenience for physicians applying for licensure in multiple states.
More than 1,963 physicians have been authenticated to use the CLAF since the State Medical Board of Ohio became the first state to integrate the new form into its online application process in December 2006. The New Hampshire Board of Medicine went live with the form in April 2007 and the Kentucky Board of Medical Licensure went live with the form in June 2007.
Developed by a workgroup of state medical board representatives, the CLAF is the first phase of the Trusted Agent pilot program, which is exploring the feasibility of building a Web-based system that will allow physicians to fill out and submit state licensing applications online, as well as obtain confidential credentialing data from primary sources in a secure environment. The Trusted Agent program is a joint effort by the FSMB and the National Board of Medical Examiners.
