Physicians An initial application fee entitles the applicant to have a personalized portfolio created and sent to one recipient. The base fee is $350 for physicians. Additional surcharges may apply to cover costs incurred to collect examination transcripts, ECFMG certification (if applicable), return of original identity document (if applicable) and other miscellaneous fees.
Subsequent Profiles (called Subsequent Requests) are forwarded for a fee of $95 for physicians for each Profile, plus applicable surcharges. To forward subsequent Profiles after an initial application has been submitted, the applicant must complete a separate Subsequent Request.
For physicians, additional profiles forwarded at the same time as an Initial or Subsequent request are only $65 each.

(A 20% discount on the initial application fee is offered to Physician Assistant applicants who submit an Initial Application within 90 days from completion of the Physician Assistant program.) A fee for return of identity document and additional surcharges may apply.

Other Fee Information and Policies
Physicians completing an application through the mail rather than completing the online application will be assessed a $50 processing fee for an Initial Application and a $25 processing fee for a Subsequent Application.
ECFMG Medical Education Verification Fee
A one-time fee of $75 is required for all international graduates for charges assessed by ECFMG to provide primary source medical education verifications to FCVS.
ECFMG Certification Status Report Fee (if applicable) For applicants certified by ECFMG, an ECFMG Certification Status Report is included in each profile forwarded to a state board. This report is required by all state boards (except Maryland) and is state-specific, proprietary, and valid for one year from issue date. A $35 fee is incurred for each state board designation.
Any document written in a language other than English that is received and/or verified by a primary source must be translated by FCVS's professional translation service unless an acceptable translation is included with the document from the primary source. FCVS does not accept translations from any other source for primary source documents. The Certification of Identification form requires the attachment of a photocopy of the proof of identity document presented to the notary. The applicant should also provide a translation of the photocopied birth certificate or passport if not in English. If an English translation is not available, the applicant may request translation by FCVS’s professional translation service (additional fee will apply).
Passport or Certified Birth Certificate shipping and handling fee Applicants submitting an original passport will incur a $25 shipping and handling fee for its return via express courier service. Applicants submitting an original birth certificate should include specific instructions if requesting return of the original document. FCVS can either return it via regular mail (no additional charge) or return it via express courier service ($25 shipping and handling fee).
Verification fee A one-time Verification fee of $25 is required by all applicants for charges assessed by institutions to provide primary source verification to FCVS.
Payment returned for insufficient funds will be assessed a $25 fee. Processing the application will be suspended until payment of the original application fee plus the $25 insufficient funds fee is received.
A significant portion of application processing occurs immediately after applications are received; therefore, requests for cancellations must be submitted in writing within five business days from the date FCVS receives the application. No refunds will be granted after five business days. In all cases of cancellation, a $50 processing fee will be deducted ($25 processing fee for subsequent applications).
Self Designation The Self designation is designed to accommodate those applicants who wish to establish their core credentials but have not yet chosen an entity to receive their profile. Applications are fully processed and the applicant receives an electronic version of their FCVS Medical Professional Profile. Please note the Self designation profile does not include an ECFMG certification report (if applicable, international medical graduates only) or examination score transcript. Profiles sent to a state medical board may require these documents and additional fees may apply.
If you are forwarding your profile to a state board or other entity and wish to receive an electronic download of your completed profile, choose an additional Self designation (additional designation fee applies).
FCVS accepts Visa, MasterCard and electronic fund transfers, and should be made within your online application. FCVS does not accept cash, check or money orders.
If an overpayment of fees is identified, FCVS will refund the credit balance at the time your profile is forwarded. Overpayment of $50 or less remaining in an FCVS account after 30 months from the application receipt date will not be refunded.
