Medical Education and Postgraduate Institutions Verification Forms
The Federation Credentials Verification Service (FCVS) now offers Medical Education and Postgraduate Institutions access to our verification forms. (*Please note: If you are a physician seeking verification of your medical credentials, please complete the FCVS application.) Click the link to send a request via email for the verification forms. Instructions on completing the verification forms are also available. These forms are editable using Adobe Acrobat Reader 7.0. Closed Residency Program Records
The Federation of State Medical Boards has offered all Accreditation Council of Graduate Medical Education - approved programs that have closed the opportunity to permanently store their resident records.
FCVS profices a central repository for these records and acts as an agent of the closed program. FCVS complies with state licensure, JCAHO, NCQA and URAC standards for primary source verification. For more information, click here.
New Service for USMLE Candidates
FCVS offers a new service to USMLE candidates who complete their Step 3 application online. As a convenience to examinees, information entered on their Step 3 online application can be used to begin a personalized FCVS Physician Information Profile that contains their primary-source verified credentials.
