General Information
Credentials FCVS Verifies
FCVS obtains direct, primary source verification of credentials in the following categories:
Identity
- Either a certified birth certificate or original passport
- Photocopy of any name change documents
- Notarized affidavit including a photograph of applicant
Academic Education
- Official transcript of undergraduate/graduate education not affiliated with the physician assistant education
Physician Assistant Program Education
- Official transcript of physician assistant program education
- Special FCVS Form, completed by the program director of the PA program
- Certified physician assistant degree diploma (FCVS will have diploma certified by the issuing institution)
Postgraduate Training
- Masters
- Residencies
- Fellowships
NCCPA Certification/Examination History
- NCCPA examination history/score report
- Certified photocopy of the physician assistant's NCCPA certificate or re-certification card
Board Action History
- FCVS obtains search results of the Federation' s Board Action Data Bank, a data bank of physician assistant disciplinary information, for each applicant. To ensure accuracy, a search is conducted each time credentials are forwarded.
