Physician Applicants to FCVS
Applicants who complete the verification process establish a permanent, lifetime portfolio of primary-source verified credentials - allowing quick, easy and inexpensive access to medical credentials. These documents can be used throughout the applicant's career for state licensure, hospital privileges, employment and professional memberships. For physicians, this service is extremely valuable for those graduating from medical schools outside the U.S. or Canada, where verification is often time consuming, costly and difficult.

Credentials FCVS Verifies
FCVS obtains direct, primary source verification of credentials in the following categories:

Identity

• Either a certified birth certificate or original passport
• Photocopy of any name change documents
• Notarized FCVS affidavit including a photograph of applicant

Medical Education

• Official transcript of medical education
• Special FCVS form, completed by the dean/principal of the medical school
• Certified medical degree diploma (FCVS will have diploma certified by the issuing institution)
• Verification of Fifth Pathway, if applicable

Postgraduate Training (US and Canada)

• Internships
• Residencies
• Fellowships
• Research

• Licentiate of the Medical Council of Canada (LMCC)

Board Action History

• FCVS obtains search results of the FSMB's Board Action Data Bank, a repository of physician disciplinary information, for each applicant. To ensure accuracy, a search is conducted each time credentials are forwarded.

FCVS Initial Application
If you graduated from a medical school that was not inside the United States or Canada, please complete the International Medical Graduate (IMG) initial application.

If you graduated from a medical school within the United State or Canada, please complete the Domestic Medical Graduate (DMG) initial application.
