Information For Credentialing Organizations
The Federation of State Medical Boards of the United States has been a recognized leader in the retention of physician data since 1912.
As postgraduate resident training programs close, the Federation offers to permanently store the records for residents having attended the program as an agent for the primary source. This ensures availability of the resident's records for future purposes of state licensure and hospital/health care privileges.
The Federation has consulted with JCAHO, NCQA, URAC and state medical boards and has determined that verification of a physician's credentials by the Federation on behalf of a closed program does meet the primary source verification requirements of each of these organizations.
How to determine if the Federation has a training program's resident records.
The introduction Web page contains a drop down list of resident training programs that have submitted records to the Federation. You may also contact a customer service agent at 1-888-ASK-FCVS.
What information is available for a resident?
Information that is available for a resident is that information which is pertinent to the credentialing verification needs of organizations such as: physician name, program name, dates of attendance, medical school affiliation, rotations, training level, training
year, department, program director name and whether or not the training was successfully completed.
How to request records from the Federation.
Click on Resident Records and Credentials Request to download the form. Complete the form with as much information as you have. Then mail the form and check or money order for $15 payable to Federation Credentials Verification Services to:
