FPDC Databank Inquiry Request
This site is only for Board Action Databank Inquiry requests by physicians or physician assistants.
If you are a physician or physician assistant who needs to have a Board Action Databank Inquiry sent directly to a state medical board for licensure purposes, please complete the request form by typing directly in the required fields. Once you have completed the form, you may save it and e-mail it to boardinquiry@fsmb.org or print the form and fax or mail to the number or address provided on the form. Please complete a separate form for each state medical board requesting a report.
If you are a physician or physician assistant who would like to have a Board Action Databank Inquiry sent to yourself, please complete the request form by typing directly in the required fields. In the field for "Name of Board", please indicate that your request is a self-inquiry. Once you have completed the form, you may save it and e-mail it to boardinquiry@fsmb.org or print the form and fax or mail to number or address provided on the form. Be sure to include the address where you would like the report sent.
