FEDERATION PHYSICIAN
DATA CENTER (FPDC)
The Federation Physician Data
Center is a central repository for formal actions
taken against physicians by state licensing and disciplinary
boards, Canadian licensing authorities, the U.S. armed
forces, the U.S. Department of Health and Human Services,
and other national and international regulatory bodies.
The Federation Physician Data Center offers two services
used in performing credentialing functions or pre-employment
background checks: the Board Action Data Search
and the Disciplinary Alert Service.
The
information contained in the FPDC Board Action Data
Bank is available to licensing and disciplinary boards,
the military, governmental and private agencies and
organizations involved in the employment and/or credentialing
of physicians.
FPDC History
For nearly a century the Federation
has collected and reported actions taken against physicians
by its member boards. In the early years, actions
were limited and were published in the Federation
Bulletin. In response to the increasing rigor and
complexity of physician regulation and to increases
in the number of physicians and board actions reported,
the Federation systematized its board action data
files in the 1960s. Today, information is processed
and distributed through sophisticated web-based servers
to provide the most effective and efficient services
possible.
FPDC Services
The Federation Physician Data
Center offers two services to assist you in your credentialing
efforts. The Board Action Data Bank Search and the
Disciplinary Alert Service initiatives are considered
primary source equivalent by NCQA, URAC and JCAHO.
- Board Action Data
Bank Search consists of a query identifying
actions reported on specific individuals. The Service
provides information pertaining to any actions contained
in FSMB's Board Action Data Bank. This service also
provides a historical view of disciplinary actions
taken by our reporting entities.
- Disciplinary Alert
Service is a continuous monitoring service
that alerts the subscriber to recently adjudicated
cases. It is a mechanism that regularly screens
practitioners against reporting entities on an ongoing
basis. This service assists subscribers with meeting
accreditation standards for the ongoing monitoring
of sanctions.
FPDC Databank Inquiry
Request
This site is only for Board
Action Databank Inquiry requests by physicians
or physician assistants. If you are
a physician or physician
assistant who needs to have a Board Action
Databank Inquiry sent directly to a state medical
board for licensure purposes, please complete the
request form by typing directly in the required fields.
Once you have completed the form, you may save it
and email it to boardinquiry@fsmb.org
or print the form and fax or mail to the number or
address provided on the form. Please complete a separate
form for each state medical board requesting a report.
If you are a physician
or physician assistant who would
like to have a Board Action Databank Inquiry sent
to yourself, please complete the request form by typing
directly in the required fields. In the field for
"Name of Board", please indicate that your
request is a self-inquiry. Once you have completed
the form, you may save it and e-mail it to boardinquiry@fsmb.org
or print the form and fax or mail to number or address
provided on the form. Be sure to include the address
where you would like the report sent.
Data
Collection & Maintenance
The reliability of physician
data begins with effective data collection. The FSMB
physician data comes from a variety of sources including
state medical boards, the Department of Health and
Human Services and a growing number of international
licensing authorities, including Canada, England,
Australia and New Zealand.
The FSMB has established comprehensive
quality control procedures to enhance the accuracy
and integrity of its information. All data is extracted
from multiple data sets and is cross-checked and matched
for accuracy with updates being provided on a regular
basis. Disciplinary sanctions and other board actions
are reported to FSMB by state medical boards and other
regulatory entities on a regular basis. These reported
actions are verified in writing and accompanied by
supporting documentation, such as copies of board
orders, findings of fact, conclusions of law, final
decrees and stipulations. Each action is reviewed
before it is added to the database.
FPDC Access
Organizations and agencies
responsible for evaluating physician applications
for licensure, employment or specific privileges access
this service. Fees for access services, when applicable,
are negotiated and stipulated by agreement executed
before services begin. Rates may be obtained from
the Physician Data Center. Eligible organizations
may request more information about accessing the Federation
Physician Data Center by completing the on-line New
User Request Form.
JCAHO/NCQA Recognition
The FSMB Disciplinary Alert
Service and the Board Action History Search have been
designated as "primary source" equivalent by the National
Committee for Quality Assurance (NCQA) and the Joint
Commission on Accreditation of Healthcare Organizations
(JCAHO) and URAC. More information on these organizations
