• Single/Double Occupancy: $216.00
• These rates are subject to 15.4% tax
Please make your reservation early, as only a limited number of rooms will be available up until the reservation cutoff date of March 30, 2010. A limited number of government rooms will also be available on a first-come, first-serve basis. The government rate is $158 per night.
Be sure to reference the FSMB Annual Meeting when booking your reservation to secure the conference rate.

Registration & Cancellations
Registration fees may be paid by check or credit card. Online registration is available for credit card payments only. To register online using a credit card, please click here. Accepted credit cards include Visa and Mastercard.
You may also register by completing and mailing/faxing the Registration Form. When paying by check, please make payable to the Federation of State Medical Boards and note 2010 Annual Meeting on your check. If paying for multiple registrations, list the full name of each attendee.
The registration fee includes admission to all Annual Meeting education sessions, meal functions and receptions. To take advantage of the early registration discount, payment must be postmarked by March 19, 2010.

Cancellations must be made in writing. Individuals who cancel before April 8, 2010 are entitled to a refund of their registration fees, less a $125 processing charge. No refunds will be provided after this date. Attendees are responsible for canceling all flight and hotel reservations.

If you're flying into O'Hare Airport, you can choose from:
- GO Airport Express - $27.00 one way; $49.00 round trip. Shuttles depart from O'Hare airport every 10-15 minutes for downtown service to Chicago hotels, downtown businesses, near north residential locations, and suburban locations. Loading zones for boarding the GO Airport Express van are at Door 1E, Door 2D, and Door 3E across from baggage claim on the Baggage Claim level. The last Airport Express van leaves O'Hare at 11:30 pm seven nights a week. "Pair Fares" (2 traveling together) and group rates (for 3 or more) are also available. If you're flying into Midway, you can choose from:

The Midway Airport Express ticket counter and loading zone is located at Door 3, lower level across from baggage claim. The last Airport Express van leaves Midway at 10:30 pm seven nights a week.
