SPEX Duplicate Score Reports
If an examinee does not receive his/her original score report, a request for a duplicate score report will be honored up to 90 days after the score report release date. The request must be made in writing to the FSMB at the address below.
If more than 90 days have passed since the score report release date, scores will be reported to the examinee only in the form of a SPEX transcript upon receipt of a signed EBAHR form and payment of the required fee.
