NOTE: Before the Board will approve an application for the USMLE Step 3, the applicant must complete the Department of Safety and Professional Services application packet, including submission of scores, as well as other documents. Licensure Application: A completed licensure application and fee must be submitted to the Department of Safety and Profession Services by July 1, 2013.

If it is determined that you do not meet eligibility requirements at the time your application is received, or you decide to cancel your registration prior to approval, or your application is not complete by the published deadline date, your registration will be cancelled and a portion of the application fee withheld. The USMLE Step 3 fee is non-refundable and non-transferable from one eligibility period to another or from one application to another.
