EXAMINATION AND BOARD ACTION HISTORY REPORT (EBAHR)
Overview
The Examination and Board Action History Report will certify a complete history of scores for a designated examination(s). The FSMB maintains scores for FLEX, SPEX and USMLE Steps 1, 2 CK & CS, and 3.
The EBAHR also includes an indication, if applicable, of any action taken and reported to the FSMB by a licensing or disciplinary board and/or other regulatory agency. FSMB considers examination scores to be confidential and, therefore, requires your authorization in order to provide an EBAHR to you or to a third party.
Transcript General Information
You may designate up to two (2) recipients for a $65 fee, regardless of the number of examinations (USMLE, FLEX, SPEX) included on the official transcript. Note: Effective January 1, 2011 EBAHR fees increased to $65 for 1-2 recipients.
An EBAHR request cannot be canceled once submitted. Additional recipients cannot be added once an EBAHR is submitted.
All EBAHR requests are processed as they are received. The Federation will not hold an EBAHR request pending the release of scores at a later date. If you have recently taken SPEX or USMLE Steps 1, 2, or 3 and need that score to appear on your EBAHR, please do not send this request until you have received your official score report for that examination.
The Federation issues official transcripts within 5 business days of receiving the completed EBAHR request and appropriate fee. Most state medical and osteopathic boards receive official transcripts in electronic format via a secure website. Please consider the table below when making a decision to pay the additional fees for express shipment. For those boards that do not receive transcripts electronically and for all other recipients (e.g., examinees, residency programs), transcripts are sent via first class mail unless express shipping is requested. Express shipping is available to addresses within the US and its territories for an additional $25 per recipient. Express service does not deliver to PO Box addresses.

Transcript Request
The Examination and Board Action History form is available online and in paper form. The online transcript request system is for transcripts designated for state medical and osteopathic boards only. If you need a transcript sent to a recipient(s) other than a state board, you MUST use the paper-based EBAHR request form.

Paper EBAHR form (for recipients other than state medical and osteopathic boards)
Note: When mailing a paper-based EBAHR form, the processing time does not begin until the request is returned from the lockbox. EBAHR forms are normally returned from the lockbox within 1-2 business days. Checks or money orders should be made payable to the Federation of State Medical Boards. A $25 fee will be charged on any returned check, and no further services from the Federation, including those from other departments, will be available until full payment is received.

Notarization
The EBAHR request form MUST be notarized in Section III, Part C. Please use the following checklist to ensure proper notarization:
- Notary's Stamp/Seal
- Notary's Name
- Notary's Signature
- Notary's Commission Expiration Date
- Date of Notarization (must be dated within the last six months)
The notary may attach an affidavit, or cover sheet, if he/she chooses. Some states require an affidavit to be used instead of notarizing the actual document. Affidavits must also meet the above checklist of requirements and be attached to the EBAHR request form. Photocopies of the notarization will NOT be accepted.

Once the EBAHR request form is completed and properly notarized, mail it, along with the appropriate payment, to the appropriate address below.
