UNIFORM APPLICATION FOR PHYSICIAN STATE LICENSURE (UA)

The Uniform Application for Physician State Licensure (UA) is a Web-based application that standardizes, simplifies and streamlines the licensure application process for physicians. Physicians fill out the UA online application once, then use the application whenever they apply for licensure in another state that accepts or requires the UA — for the rest of their careers.

 Note:  On October 27, 2011, the FSMB began collecting a one-time service charge of $50.00 from all Uniform Application (UA) applicants when they submit the UA for the first time. This service charge is collected by the FSMB, not state boards. This charge is separate from any state licensure application fees.

Why use the UA? The UA:

  • Apply for licensure in one or more statesStreamlines the process for physicians applying for licensure in multiple states
  • Can easily be updated and reused by physicians whenever they apply for licensure in a new state
  • Auto-populates credentialing data for users of the Federation Credentials Verification Service (FCVS)

The Uniform Application streamlines and simplifies applying for a medical license, allowing more time with patients and less with paperwork.
