 Reminder: Deadline Dates for the 2012 USMLE Step 3 Applications and Supporting
Documents 
All applications for the USMLE Step 3 - 2012 application cycle must be received by August 31, 2012 at 5 pm Central Time. All supporting documents (i.e., Certification of Identity forms, post graduate training forms and board approvals from ID, IL, KS, LA, MI, MS, NY) must be received not later than September 7, 2012 at 5 pm Central Time. Applicants for AL, DC, MO and WI should contact the state board to know that status of their Step 3 application.

USMLE Step 3 Fees and Forms
The fee for the 2011 Step 3 is $730 for all state medical boards with the exception of those boards listed below.

  • Iowa $780
  • South Dakota $880
  • Vermont $765

The USMLE Step 3 fee is non-refundable and non-transferable from one eligibility period to another or from one application to another.

If applying online, your fee must be paid by credit card.

If mailing your Step 3 application, you will need to submit a check or money order made payable to FSMB in US currency. Record your name, USMLE # and social security or national identification number on your check or money order.

It is recommended that you send your application materials and fee by a traceable means such as US Postal Service certified mail return receipt or an overnight courier. Documents sent via the United States postal service with regular mail can not be tracked.

USMLE Step 3 | Online Application
To utilize the FSMB's online registration for Step 3 you will need:

  • ACCESS TO A COMPUTER WITH A PRINTER: The computer must have one of the following web browsers installed: Microsoft Internet Explorer 5.x or Netscape Navigator 6.x or higher. The printer is necessary to print and then mail the required forms, e.g., the Certification of Identity form.
  • A CREDIT OR DEBIT CARD: A Visa or Master Card will be necessary to submit payment for the Step 3 Online Application.
  • AN E-MAIL ADDRESS: This will be the primary means for FSMB to communicate with you regarding your application. An email address must be provided on your Step 3 application. This email will be used to notify you of your Step 3 application processing status. You must make sure that you allow emails from usmle@fsmb.org to be received by your email address. Examinees that provide an email address ending in an .edu and some free accounts will either reject the email notifications all together or will send the email to your SPAM or JUNK email folders. Non receipt of emails will not be accepted to request extensions or refunds for the Step 3.

If you do not have each of these items listed above, you will have to complete a paper application.

Before you begin the online registration process, you should carefully review both the USMLE Bulletin of Information, the USMLE Step 3 Content Description and General Information and the Biometric Identity Management System Privacy Notice for USMLE Candidates. Your signature on the Certification of Identity form signifies that you meet all USMLE and state-specific eligibility requirements, have read the Bulletin of Information and agree to abide by the policies and procedures it describes.

At the end of your registration for the USMLE Step 3 online application you will be asked if you would like to create an FCVS profile using your Step 3 information. This is strictly voluntary. If you are going to apply for licensure with a board that is an "accepting or requiring " board of FCVS profiles, it may be beneficial to you to create your FCVS profile now. [ FAQ on FCVS ]

 

USMLE Step 3 | Paper Application
Before printing and submitting the paper application, there are several things you should do to ensure a smooth experience in registering for Step 3.

  • You should carefully review the USMLE Bulletin of Information, the USMLE Step 3 Content Description and General Information and the Biometric Identity Management System Privacy Notice for USMLE Candidates. Your signature on the Certification of Identity form signifies that you meet all USMLE and state-specific eligibility requirements, have read the Bulletin of Information and agree to abide by the policies and procedures it describes.
  • Ensure that you meet the USMLE program's eligibility requirements for taking Step 3.
  • In completing the application, you will be asked to specify the state medical board to for which you are sitting Step 3. Most state medical boards have additional state-specific criteria for sitting Step 3, e.g., a minimum postgraduate training requirement, attempt limits and/or time limits for completing the USMLE.
  • An Email Address:This will be the primary means for FSMB to communicate with you regarding your application. An email address must be provided on your Step 3 application. This email will be used to notify you of your Step 3 application processing status. You must make sure that you allow emails from usmle@fmsb.org to be received by your email addresses. Examinees that provide email addresses ending in an .edu and some free accounts will either reject the email notifications all together or will send the email to your SPAM or JUNK email folders. Non receipt of emails will not be accepted to request extensions or refunds for the Step 3.

Consult the State-specific Requirement Chart to determine which state's eligibility criteria you meet. Then print the instructions for that state for future reference.

The 2011 USMLE Step 3 Application form and Instructions are in addition to any state specific instructions obtained from the State-specific instructions and are required for all applicants.

Notary Guide
When having your Certification of Identity form or other documents (e.g., diploma, 5th Pathway Certificate, postgraduate training form) notarized, please use the following checklist as a guide to ensure proper notarization.

All documents require the following:

  • Notary's Stamp or Seal
  • Notary's Name
  • Notary's Signature
  • Notary's Commission Expiration Date (if no expiration date must provide explanation)
  • Date of Notarization (Must be original and dated within the last six months)

Documents that must be certified require the notary to certify that the document is a "true and correct copy of the original". If the notary will not certify the document, you may attest that it is a "true and correct copy of the original" and sign the statement. The notary may then notarize your signature.

If your document is not in English or Latin, it must be translated into English. The translation and the copy of the original language document must both be notarized and submitted. Please submit only photocopies of any required document as these documents will not be returned. Photocopies and faxes of the notarization will not be accepted.
