If are unable to take the test within your original eligibility period you can request a one-time contiguous 90-day extension.

Applicants requesting an eligibility period extension must download and mail the Eligibility Extension form to FSMB offices to an address that will be provided on the form.

The Eligibility Extension fee is $55. The Eligibility Extension form and fee must be received in our office no later than 10 days after the expiration of the original eligibility period's end date.
