MEDICAL LICENSING EXAMINATION

USMLE Step 3 Forms
The following forms may be required for your USMLE Step 3 application. See the tips pages for instructions on proper notarization.

• Certification of Identity (CID) Form - required for all applications unless one is already on file.
• Postgraduate Training (PGT) forms.
  • A state-specific PGT form is required for applicants registered to sit for Step 3 in:
    • Hawaii
    • Massachusetts
    • Michigan
    • Ohio
    • Pennsylvania
  • Applicants for all other boards that require a PGT form can use the generic PGT form.
• Georgia: Attestation of Medical Training form
• Michigan: Michigan Specific Application for USMLE Step 3
• New Jersey: Certification of Pre-Medical Education form

Use these forms to amend your application:

• Eligibility Extension
• Name Change/Correction Authorization Form

Visit the Test Accommodations page on the USMLE website to download the forms to:

• Request new test accommodations or to request a change to accommodations received previously
• Request the same accommodations for Step 3 that you receiving on Step 1 or Step 2
