Note: If you are an international medical graduate (IMG) who completed a 5th Pathway program, you will need to mail a notarized copy of both your medical school diploma and the 5th Pathway certificate directly to FSMB. These will be required to complete your application once the state medical board forwards your Step 3 application to FSMB for final processing.
