Practitioner Direct

Getting your credentials to a state medical board shouldn't be complicated. Practitioner Direct lets you send your Physician Data Center (PDC) Profile or National Practitioner Data Bank (NPDB) report directly and securely, with no extra steps on your end. 

How It Works

Practitioner Direct covers two types of reports: Your PDC Profile contains licensure and disciplinary information that may be required as part of your state medical licensure application. An NPDB report records either a medical malpractice payment paid on behalf of a health care practitioner or an adverse action taken against a health care practitioner, organization, provider, or supplier.  There is no fee to submit either report through Practitioner Direct.

SENDING YOUR PDC PROFILE
State medical boards may use different names for the PDC Profile, such as a board action clearance report, licensure verification report, or disciplinary history report. Regardless of the name used, the PDC Profile provides the information the state medical board needs to verify your licensure history and review any reported board actions or disciplinary history. If your medical board requires this information, Practitioner Direct sends your PDC Profile straight to them electronically.

REQUESTING A SELF-QUERY
You may also request that your PDC Profile be sent directly to you. A self-query allows you to review the information included in your profile before or during the licensure application process.

SENDING AN NPDB REPORT
Some state medical boards also require an NPDB report. If yours is a participating board, Practitioner Direct sends it electronically, so you never have to receive or mail a paper copy yourself.

WHAT IS NOT INCLUDED
Your PDC Profile does not include your USMLE examination scores. To request a copy of your USMLE scores, visit the USMLE Score Transcript Service.
