For state medical boards requiring FCVS profiles, the Uniform Application is included at no additional charge.

The UA has a one-time, nonrefundable fee of $60 for the initial application of each MD, DO or PA license for state medical boards that do not require FCVS profiles. There is no fee for additional applications.

The UA does not include your credentials or an official transcript of your examination scores. These documents must be requested separately.

To use the UA, visit FSMB's Physician Services Portal, select “Uniform Application,” and sign in as directed.

In addition to completing the core fields of the UA online:

• • Refer to the state-specific instructions included in the UA.
  • Submit notarized UA Affidavit and Authorization for Release of Information forms to the state medical board. The UA Affidavit is separate from the FCVS Affidavit and must be sent to the medical board, not to FSMB.
  • Have each license you’ve held verified by the medical board of each granting state. You can determine the fees and verification method for each board by using the licensure verification resource. Use the UA Licensure Verification Form for boards that require a written request. If the board uses VeriDoc or another method, use the preferred method instead of the UA form.
